Reestructuración de los focos terapéuticos de una consultante en un proceso de psicoterapia dinámica interpersonal breve y su relación con la presencia de sintomatología depresiva

Abstract

La presente investigación tuvo como objetivo describir y analizar la evolución de los focos terapéuticos según el sistema OPD y su relación con la intensidad de la sintomatología depresiva en una paciente que participa en la terapia dinámica interpersonal breve (DIT). Para ello, se analizaron los episodios relacionales de las tres primeras sesiones con la finalidad de identificar los focos terapéuticos según el OPD-2, Luego, se analizaron las transcripciones de las doce sesiones completas del DIT para evaluar el nivel de integración de los focos según la escala de reestructuración de Heidelberg (HSCS). Finalmente, se obtuvo el reporte de la sintomatología depresiva según la escala PHQ 9 y se comparó con la trayectoria de la reestructuración de los focos terapéuticos. Los resultados evidencian que el foco relacional obtuvo un nivel de reestructuración con tendencia creciente, mientras que los focos de conflicto y estructura se mantuvieron en un nivel de manejo por parte de la consultante. Así mismo, el nivel de sintomatología depresiva se fue reduciendo a lo largo de las sesiones, lo cual parece estar vinculado con una mayor reestructuración del foco relacional.

The aim of this research was to describe and analyze the evolution of therapeutic foci according to the OPD system and its relationship with the level of depressive symptoms in a patient participating in brief dynamic interpersonal therapy (DIT). To do this, the relational episodes of the first three sessions were analyzed in order to identify the therapeutic foci according to the OPD-2. Then, the transcripts of the twelve complete sessions of the DIT were analyzed to evaluate the level of integration of the foci according to the Heidelberg restructuring scale (HSCS). Finally, the report of depressive symptoms was obtained according to the PHQ 9 and it was compared with the trajectory of the restructuring of the therapeutic foci. The results show that the relational focus obtained a level of restructuring with an increasing trend, while the conflict and structure focuses remained at a level of management by the consultant. Likewise, the level of depressive symptoms was reduced throughout the sessions, which seems to be linked to a greater restructuring of the relational focus.
