Abstract
El presente trabajo de investigación se basa en un proyecto de innovación para mejorar la
disponibilidad de las Historias Clínicas y así reducir el tiempo de espera de los usuarios
externos para la atención en Consulta Externa en el “Hospital Regional Docente de
Cajamarca”. Por ende, se determinó el problema y sus causas como el déficit de equipo
emisor de tickets, personal poco capacitado, atención por orden de llegada de los usuarios
externos, Historia Clínica no disponible para la atención en Consulta Externa, y la
ausencia del médico en el consultorio externo. Este trabajo se enmarca dentro de la
investigación aplicada y buscar generar innovación ante una problemática. Se realizó una
jerarquización de las causas en base a criterios referidos al nivel de impacto en el
problema encontrado, modificación y si se encontraba en el ámbito normativo de la
Organización, desde el cual pretende generar la intervención. El problema fue
reformulado y de esta manera, el desafío de innovación se convirtió en ¿Cómo podemos
mejorar la disponibilidad de las Historias Clínicas de los usuarios externos para disminuir
el tiempo de espera para la atención en Consulta Externa del Hospital Regional Docente
de Cajamarca? El prototipo de innovación está compuesto por un eje central que es el
“Aplicativo informático de seguimiento de las Historias Clínicas”, y de cuatro
componentes como la capacitación sobre manejo de Historias Clínicas, el reordenamiento
físico de Historias Clínicas, Directiva Administrativa de Procesos de gestión de Historias
Clínicas, y las capacitaciones sobre salud dirigido mejorar el tiempo de espera subjetivo
de los usuarios externos en Consulta externa. Finalmente, después de testear el prototipo
en el hospital, además de cumplir con los análisis de deseabilidad, factibilidad y de
viabilidad, se concluye que el referido prototipo generará valor público, relevancia social,
utilidad práctica y sobre todo un mayor beneficio a los ciudadanos.
The present research work is based on an innovation project to improve the availability of Clinic Records and thus reduce the waiting time of external users for care in outpatient at the “Regional Teaching Hospital of Cajamarca”. Therefore, the problem and its causes were determined such asthe deficit of equipment issuing tickets, poorly trained personnel, attention by order of arrival of external users, Clinical History not available for attention for care outpatient consultation, and the absence of the doctor in the outpatient clinic. This work is part of applied research and seeks to generate innovation in the face of a problem. A hierarchy of the causes was made based on criteria referring to the level of impact on the problem found, modification and if it was found in the normative scope of the Organization from which it intends to generate the intervention. The problem was reformulated and, in this way, the innovation challenge became: How can we improve the availability of the Clinical History of external users to reduce the waiting time for care in Outpatient Department of the Regional Teaching Hospital of Cajamarca? The innovation prototype is made up of a central axis that is the "Computer application for monitoring Clinical Histories", and four components such as training on the management of Clinical Histories, the physical rearrangement of Clinical Histories, Administrative Directive of Management Processes of Clinical Histories, and health training aimed at improve the subjective waiting time of external users in Outpatient Consultation. Finally, after testing the prototype in the hospital, in addition to complying with the analyzes of desirability, feasibility and viability, it is concluded that the prototype will generate public value, social relevance, practical usefulness and above all a greater benefit to citizens.
