Factores que limitan la gestión del Seguro Integral de Salud (SIS) para la disminución del gasto de bolsillo en la atención de niños y niñas referidos al Hospital Subregional de Andahuaylas, Apurímac, 2020-2022

Resumen
El objetivo de la investigación fue identificar los factores que limitan la gestión del SIS
para la disminución del gasto de bolsillo en la atención de niños y niñas referidos al
Hospital Subregional de Andahuaylas (HSRA), Apurímac, 2020-2022.
Se ha analizado a) la capacidad de gestión y habilidades del SIS para lograr sus
objetivos, b) la ejecución y logros del SIS, c) el efecto del gasto de bolsillo en salud en
la familia de los beneficiarios; así como d) la valoración y satisfacción de los beneficiarios
acerca del SIS. Finalmente, se formula una propuesta de mejora en la gestión del SIS
para la atención de salud y disminución del gasto de bolsillo de los beneficiarios.
La estrategia metodológica fue mixta, combinando la metodología cualitativa y
cuantitativa. Se entrevistó al personal de salud, administrativo y beneficiarios,
revisándose los documentos y la observación de la atención de los beneficiarios.
Se concluye que los factores limitantes de la gestión del SIS para la disminución
del gasto de bolsillo de los usuarios referidos al HSRA son la deficiente infraestructura,
insuficiente equipamiento y escasos recursos humanos. Otros factores limitantes fueron
el Sistema Informático de EsSalud no actualizado que impide que los beneficiarios sean
atendidos por el SIS. Llenado inadecuado de los formatos de referencia y atención del
SIS. No disponibilidad de personal exclusivo para información y orientación de los
beneficiarios. El Servicio de farmacia se encontró desabastecido y con medicamentos
fuera del petitorio, esta es otra limita al SIS. Beneficiarios con desconocimiento de los
procesos de atención en el hospital y personal de salud y administrativo no capacitado
en temas específicos del SIS. Se encontró que los tiempos de espera son muy
prolongados para la atención de la salud de los niños y niñas; además se presentó poca
disponibilidad de citas mensuales, limitada programación de médicos pediatras para
atención en consultorio y deficiente atención telefónica para citas con referencia.
The objective of the research was to identify the factors that limit the management of the SIS to reduce out-of-pocket spending in the care of boys and girls referred to the Andahuaylas Subregional Hospital (HSRA), Apurímac, 2020-2022. The analysis of the management capacity and skills of the SIS to achieve its objectives, the analysis of the execution and achievements of the SIS and the effect of out-of-pocket health spending on the beneficiaries' families has been carried out; as well as the evaluation and satisfaction of the beneficiaries about the SIS intervention. Finally, a proposal is formulated to improve the management of the SIS for health care and reduce the out-ofpocket expenses of the beneficiaries. The methodological strategy was mixed, combining qualitative and quantitative methodology. Semi-structured interviews were carried out with health, administrative and beneficiary personnel, documentary review and observation of the beneficiaries' care. It is concluded that the limiting factors of SIS management to reduce out-of-pocket spending in beneficiaries referred to the HSRA are: HSRA with problems of poor infrastructure, insufficient equipment and human resources. EsSalud computer system not updated in a timely manner. Inadequate filling out of the SIS reference and attention forms. Non-availability of exclusive personnel for information and guidance of beneficiaries. Pharmacy service with some medications in stock and prescription of medications outside the request. Beneficiaries with ignorance of the care processes in the hospital. Health and administrative personnel not trained in specific SIS topics. Long average waiting times for care. Little availability of monthly appointments and limited scheduling of pediatric doctors for in-office care and poor telephone service for appointments for reference of beneficiaries. Finally, based on the analysis of the results of this research, an application proposal is presented, “Comprehensive Plan to improve care and reduce out-of-pocket expenses for SIS beneficiaries referred to the HSRA”, which allows us to address these limitations and propose viable improvements.
