Creencias sobre una violencia (in)visible: Experiencias de violencia obstétrica en mujeres usuarias de hospitales de Lima Metropolitana

Abstract

La violencia obstétrica (V.O) es un fenómeno reconocido como obstaculizador al derecho a acceder a una salud sexual y reproductiva digna; ello por su perjudicial impacto en el bienestar de las mujeres durante el proceso del embarazo, parto y post parto (OMS, 2014). Buscando visibilizar esta problemática en el contexto peruano, la presente investigación tuvo como objetivo explorar las creencias sobre la experiencia de violencia obstétrica durante el proceso de parto de usuarias de hospitales públicos de Lima Metropolitana. Para ello, se llevó a cabo un estudio cualitativo exploratorio. Se halló creencias sobre la construcción del paciente desde un rol de pasividad y desconocimiento; en cambio, la figura del personal médico fue reconocida por su rol desestabilizador y de apoyo autoritario. Al presentarse V.O en el parto, las participantes manifestaron un rotundo rechazo a este tipo de atención, sin embargo, simultáneamente, se identificó que es una experiencia que puede ser naturalizada al minimizarse manifestaciones indirectas del fenómeno. Se instrumentalizó estas acciones al considerarlas como facilitadoras en el éxito del parto en pacientes no colaborativas. Asimismo, se identificó creencias de haber sido representadas desde la inferioridad, despersonalización y obediencia. Ante experiencias de V.O, se presentaron afrontamientos desde la pasividad, evitación y resistencia. Se calificó la situación vivida como inevitable pero perjudicial e injusta para las mujeres que se atienden en servicios de salud pública. Durante el diálogo, se manifestó un proceso de resignificación a partir de la búsqueda de visibilizar y prevenir la ocurrencia de violencia obstétrica.

Obstetric violence (OV) is a phenomenon recognized as an obstacle to the right to access to appropriate sexual and reproductive healthcare; this due to its negative impact on the well-being of women during the process of pregnancy, childbirth and postpartum (WHO, 2014). In order to highlight this issue in the Peruvian context, this study aimed to explore the beliefs about the experience of obstetric violence during the delivery process of women in public hospitals in Metropolitan Lima. For this purpose, exploratory qualitative research was carried out, results show that beliefs about the figure of the patient were constructed from a role of passivity and ignorance, on the other hand, the figure of the health care providers was recognized for its destabilizing and authoritarian support. Although the presence of violence in childbirth was rejected by the participants, this study found that indirect manifestations of violence were naturalized and instrumentalized by minimizing these expressions and considering them useful to facilitate childbirth in non-cooperative patients. Likewise, the results show that women believe that they were represented from narratives of inferiority, depersonalization and obedience. In the face of V.O experiences, coping mechanisms from positions of passivity, avoidance and resistance were identified. This type of mistreatment was classified as inevitable but harmful and unfair for female users of public health services. Lastly, a process of resignification of the experience of maternal health care was manifested from the search to raise awareness and prevent the occurrence of this problem.
