Significados atribuidos a la muerte y la experiencia de duelo por pérdida de pacientes en residentes y asistentes de cirugía pediátrica de Lima Metropolitana

Abstract

La presente investigación buscó explorar los significados atribuidos a la muerte y la experiencia de duelo por pérdida de pacientes en residentes y asistentes de cirugía pediátrica de hospitales públicos de Lima Metropolitana. Ello desde una metodología cualitativa a través de ocho entrevistas semiestructuradas y un análisis temático inductivo. Los resultados se agruparon en dos temas. El primero describe elementos contextuales como la cultura organizacional médica, la falta de un enfoque tanatológico en la formación y la escasez de recursos que pueden interferir negativamente en el duelo. Igualmente, presenta a la muerte como el fin inevitable de la vida, pero como una enemiga a vencer en la práctica médica como significados coexistentes en las personas participantes. El segundo tema presenta la vivencia de un duelo desautorizado de acuerdo con el vínculo mantenido, las manifestaciones identificadas; así como el uso de estrategias de afrontamiento enfocadas en evitación y emoción. Transversalmente, se destacaron divergencias en función al género y al tiempo de práctica clínica en las cuales los participantes varones y con menos tiempo en dicha práctica tendrían mayores dificultades para experimentar el duelo libremente. Este estudio contribuye a la comprensión del duelo en contextos sanitarios, cuyos resultados resaltan la necesidad de intervenir en la desestigmatización del duelo, así como en las barreras estructurales de la salud pública para mejorar el bienestar y desempeño de estos y estas profesionales.

This study aimed to explore the meanings attributed to death and the grief experiences following patient loss among pediatric surgery residents and attending physicians at public hospitals in Lima. This study employed a qualitative methodology using eight semi-structured interviews and an inductive thematic analysis. The findings were grouped into two themes. The first describes contextual elements such as the medical organizational culture, the lack of a thanatological approach in medical training, and the scarcity of resources that may hinder the grieving process. It also highlights two coexisting meanings of death among participants: one as the inevitable end of life, and another as an adversary to be defeated in medical practice. The second theme addresses the experience of disenfranchised grief, shaped by the nature of the professional bond with the patient, the manifestations of grief, and the use of coping strategies focused on avoidance and emotion. Cross-cutting differences emerged based on gender and years of clinical practice, with younger male physicians showing greater difficulty in freely experiencing grief. This research contributes to the understanding of grief in healthcare contexts, underscoring the need to address both the stigmatization of grief and the structural barriers in public health to improve healthcare workers' well-being and performance.
