Caso clínico Mirella

Abstract

El presente caso clínico es sobre Mirella, una joven de 18 años que llega a consulta buscando descargar sus emociones sin que la juzguen y la entiendan. El objetivo de este espacio de consejería, el cual constó de 5 sesiones, fue intentar que ella pueda cuestionarse si realmente podría confiar en alguien ya que hubo varias experiencias en su vida que la llevaron a un dolor psíquico asociado a la percepción de mentira y el juicio. Para el desarrollo del análisis se han utilizado a diversos autores que mencionan sobre aproximaciones psicoanalíticas teóricos técnicas y se consideró algunas características esenciales en el marco analítico, las cuales se flexibilizaron en el encuentro con la paciente. El tipo de relación objetal que domina su mundo interno es la parcial ya que no puede integrar que una persona tiene aspectos positivos y negativos, sino que percibe a su entorno muy amenazante y la angustia que predomina es la paranoide porque su Yo tiene miedo a aniquilarse y expulsa hacia afuera lo negativo. Entre sus mecanismos de defensas está la escisión, la proyección y la represión de sus emociones. Mirella cuenta con recursos como la capacidad de reflexión, comprensión sobre las opiniones de otros y una actitud positiva para la sesión; y podría continuar con una terapia psicoanalítica para seguir elaborando los diversos temas que pueda traer pero debería fortalecer su compromiso terapéutico, el cual le permitiría ser capaz de involucrarse, confiar y tolerar la intimidad del proceso.

This clinical case is about Mirella, an 18-year-old girl who comes to the clinic looking to unload her emotions without being judged and misundertood. The objective of this counseling space, which consisted of 5 sessions, was to try to make her question if she could really trust someone, as there were several experiences in her life that led her to psychic pain associated to the perception of lying and judgment. For the development of the analysis the work of various authors has been used, work that mentions about technical theoretical psychoanalytic approaches and some essential characteristics were considered in the analytical framework, characteristics which were made more flexible in the encounter with the patient. The type of objectal relationship that dominates her internal world is partial, since she cannot integrate that a person can have positive and negative aspects, but instead perceives her environment as very threatening and the distress that prevails is the paranoid one because her Ego is afraid of annihilating itself and expels the negative outwards. Among her defense mechanisms is splitting, projection, and repression of her emotions. Mirella has resources such as the capacity for reflection, understanding of the opinions of others and a positive attitude for the session and she could continue with psychoanalytic therapy to keep up working on the various subjects that she may bring up, but she should strengthen her therapeutic commitment, which will allow her to be capable to engage, trust and tolerate the intimacy of the process.
